I've considered a theory that superheroes were some sort of covert branch of the military -- with those below the rank of captain undergoing intensive training, and those above involved in management and strategic planning and so on. But a superhero called "Captain" in a covert operation would sort of give the game away, wouldn't it?
More likely, "Captain" was once a popular given name, like "Fred" or "Nathaniel." One example might be Captain Kangeroo.
